Good Faith Estimate Notice
Under the No Surprises Act, health care providers are required to provide clients who are not using insurance with a Good Faith Estimate of the expected cost of services.
Your Right to a Good Faith Estimate
New Leaf Counseling is a self-pay practice and does not bill insurance. Upon request, you will receive a Good Faith Estimate outlining the expected cost of services before treatment begins.
A Good Faith Estimate explains the anticipated charges for non-emergency health care services, including psychotherapy services. You may request a Good Faith Estimate before scheduling a service or at any time during treatment.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
Please keep a copy of your Good Faith Estimate for your records.
